TO OBTAIN A CERTIFIED COPY OF A BIRTH OR DEATH
CERTIFICATE AN APPLICANT MUST MEET THE FOLLOWING CRITERIA: |

The applicant must be the individual applying, or an immediate family member, a
legal representative or a properly qualified person of the applicant. |

Birth Certificate Certified Copy Fee = $21.00
|
|
|
|
Death Certificate Certified Copy Fee = $19.00 + $3.00 for each additional copy of
same record.

|

APPLICATIONS WITHOUT PHOTO IDENTIFICATION WILL NOT BE PROCESSED.
